Position is contingent upon award 

A professional who performs the initial interview to recognize and track symptoms, provide
educational resources and guidance and obtain relevant contacts.

Tasks and Responsibilities may include all the following, but not limited to:
•Prepares case interviews for infected patients received from reporting sources, state and local health departments, or similar Tribal programs (such as Tribal Epidemiology Center (TEC) or Tribal Health Department) or urban Indian organizations (UIO)
•Conducts patient interviews to identify symptoms to determine infection rate and potential risks
•Obtains all frequent or recent contacts and locations visited
•Maintains appropriate discretion with the information
• Provides educational materials when necessary
•Documents case findings and relaying information to identified local contact or authority
•Initial call/case investigation/contact tracing, provide some oversight, coordinating effort of the Contract Tracers/Public Health Specialists

Minimum Qualifications/ Experience Requirement:
•Health care, public health or related experience
•Case Management experience

Preferred Qualifications / Experience Requirement:
•Qualified candidate who is local to the community or reservation
•Qualified candidate who is fluent in the major Native language of the local community or Reservation
•Candidate who can provide onsite support
•Experience in using RPMS or other Electronic Health Record System
•RN, preferably a PHN
•Candidate with MPH degree
•Bilingual – Navajo, Western Apache, Yavapai, Havasupai-Hualapai, Quechan/Yuma, Mojave, Maricopa, Cocopah, Hopi, Colorado River Numic, O’odham, Yaqui, Halchidhoma


 Job Summary

Essential Duties and Responsibilities:
- Administer and process all incoming medical records and lay evidence included with a proposal.
- Ensure correct documents are provided to the reviewer.
- Review documents determining completeness and eligibility prior to assigning to the reviewer.
- Examine case file to ensure all relevant information has been submitted.
- Contact appropriate parties for required documents and/or clarification, if not received.
- Succinctly summarize the facts for each case for the reviewer indicating what medical evidence is present supporting the set-aside proposal.
- Work closely with SMEs to develop and implement internal procedures, policy communications, work flows, QC documents, training materials and job aids.

Minimum Requirements:
- High School diploma or equivalent with 1-2 years of experience.
- Associate degree preferred.
