Job Description

OPPORTUNITY LOCATED IN QUEENS, NY

The Navigator serves as a client advocate for the New York City Human Resources Administration WeCARE program. This position will meet with all clients as the FCO is completed. The Navigator will serve an advocate for the client in all stages of the WeCARE program. The Ideal Candidate will Possess the Following Additional Education and Experience
  • Meet with the client upon completion of the FCO, provide a clear and understandable explanation of the FCO to the client.  Navigator will listen to understand the client issues, and advocate for the client within the program.
  • For clients who do not agree with their FCO, they will facilitate a medical evaluation
  • Will discuss accommodations with client, document the need for accommodations and facilitate receipt of accommodations
  • Will initiate service plans for each client that is assigned to a service track, and will introduce the client to their new case manager
  • Will act as an advocate and be available for any WeCARE client that has moved to a service track
  • Provide supervisors with trends they are seeing that have a negative impact on the quality of service the client is receiving from WeCARE
  • Performs other duties as may be assigned by management
  • Must have strong communication skills
  • Strong Customer Service skills
  • Listen in a non-judgmental manner
  • Be an effective advocate
  • Proficient in Microsoft Word, Excel, PowerPoint, etc.
  • Knowledge of Community Resources
  • Able to manage multiple tasks
  • Perform other duties as may be assigned by management

EDUCATION / EXPERIENCE:

a) a bachelor’s degree in social work or a related field, such as psychology, vocational rehabilitation or occupational rehabilitation from an accredited institution and one (1) year experience in case management; or b) training and/or work experience determined to be acceptable by the Agency and would  give preference to potential non-supervisory staff with relevant subject matter experience.
